Your plan web page makes it easier to find the plan materials you need. It includes information like:

• A video that walks you through important plan details like costs and coverage.
• Plan materials like your Summary of Benefits, formulary (drug list) and Evidence of Coverage.
• Easy access to links that can help you find a provider or dentist and pay your premium. Or get an over-the-counter catalog, Member Handbook and forms like the reimbursement form.
